What are custom orthotics?
Custom orthotics are specialized shoe inserts that are designed to support and correct the alignment of the feet. They are often prescribed by a healthcare professional to address foot problems such as flat feet, high arches, plantar fasciitis, and other conditions that can cause pain or discomfort.
Custom orthotics are made from molds of the patient’s feet and are designed to fit the specific contours of the foot. They can be made from a variety of materials, including foam, plastic, and leather, and are typically placed inside the shoe to provide additional cushioning and support. The process of getting custom orthotics involves a consultation with a healthcare professional, who will evaluate the patient’s foot structure and discuss any symptoms or concerns.
If custom orthotics are deemed appropriate, the healthcare professional will take a mold of the patient’s feet, which will be used to create the customized inserts. Once the custom orthotics are made, the patient will typically be instructed on how to properly wear and care for them. With proper use, custom orthotics can provide significant relief for foot pain and help improve overall foot function.
Overall, custom orthotics can be a beneficial treatment option for anyone looking to improve their foot function, reduce pain or prevent foot injuries. However, it’s important to consult with a healthcare professional to determine if custom orthotics are the right treatment option for your specific needs. – Dr. Parm Bisla

There are several different methods for making custom orthotics, including foam casting and 3D scanner orthotic casting. Here’s a brief overview of each process:
Foam Casting:
- The healthcare professional will have the patient stand or sit with their feet in a neutral position.
- The healthcare professional will apply a layer of foam to the bottom of the patient’s foot.
- The patient will then be instructed to step down onto a foam box, which will leave an impression of their foot in the foam.
- The foam box with the impression will be sent to a lab where a mold of the patient’s foot will be made from the impression.
- The custom orthotics will be made from the mold and sent back to the healthcare professional to be fitted and adjusted as needed.
3D Scanner Orthotic Casting:
- The healthcare professional will use a 3D scanner to capture a digital image of the patient’s foot.
- The healthcare professional will then use software to create a 3D model of the foot based on the digital image.
- The 3D model will be used to design the custom orthotics.
- The custom orthotics will be manufactured using a 3D printer or a CNC (Computer Numerical Control) machine.
- The custom orthotics will be sent back to the healthcare professional to be fitted and adjusted as needed.
- Both foam casting and 3D scanner orthotic casting can produce accurate and effective custom orthotics. The choice of method often depends on the preference of the healthcare professional and the specific needs of the patient.
- Athletes: Custom orthotics can help improve athletic performance by providing better support and alignment, reducing the risk of injury, and improving overall foot function.
- Individuals with foot conditions: Custom orthotics can help alleviate pain and discomfort caused by a variety of foot conditions, including plantar fasciitis, flat feet, high arches, bunions, and hammer toes.
- Individuals with over-pronation or supination: Custom orthotics can help correct foot pronation and supination issues by providing additional support and cushioning.
- Individuals with jobs that require standing or walking for long periods: Custom orthotics can help reduce foot fatigue and discomfort associated with jobs that require prolonged standing or walking.
- Individuals with a history of foot injuries: Custom orthotics can help reduce the risk of foot injuries by providing additional support and cushioning, especially for individuals with a history of foot injuries.
- Individuals with diabetes or other conditions that affect foot health: Custom orthotics can help improve foot health and reduce the risk of complications for individuals with diabetes or other conditions that affect foot health.
Yes, we do offer medical grade compression sleeves. They are similar to compression socks, but instead of covering the entire foot and calf, they only cover the lower leg, from the ankle to just below the knee. Medical grade compression sleeves provide graduated compression, which means the pressure is highest at the ankle and gradually decreases up the leg. This helps to improve blood flow, reduce swelling, and prevent blood clots in the lower leg.
Medical grade compression sleeves are often used as a treatment for a range of medical conditions, such as varicose veins, lymphedema, and deep vein thrombosis (DVT), and are available in various compression levels and sizes to suit individual needs. Medical grade compression sleeves can also help improve athletic performance by reducing muscle fatigue and soreness, improving blood flow, and preventing injuries such as shin splints.
It’s advisable to confirm your coverage with your extended health plan as the extent of coverage for custom orthotics can vary depending on the specific plan. However, in most cases, the majority or even the entire cost of custom orthotics is covered by these plans.
Fisiomed Mobile Clinic is not currently able to direct bill extended health plans for custom orthotics. As a result, you will need to pay for your custom orthotics upfront and then submit the expense to your extended health plan for potential reimbursement, depending on your specific plan’s coverage.
However, if you have been injured and require custom orthotics as part of your treatment plan, ICBC and WorkSafe may cover the cost of your orthotics. In such cases, we are able to directly bill ICBC or WorkSafe for the cost of your custom orthotics, provided there is a medical need for them and they are deemed necessary to improve your condition following your injury.
While you don’t need a doctor’s note to undergo a biomechanics assessment for custom orthotics, it’s worth noting that many extended health plans require a doctor’s prescription for custom orthotics when you submit your receipt for reimbursement.
Therefore, we recommend obtaining a doctor’s prescription prior to scheduling a biomechanics assessment with one of our healthcare professionals. This will ensure that you have all the necessary documentation to submit a claim to your extended health plan and increase the likelihood of receiving reimbursement for your custom orthotics.
Following your biomechanics assessment, we will send the impressions of your feet to our partnered lab, where skilled technicians will create unique orthotics designed specifically for your feet and condition. Typically, the turnaround time for the creation of custom orthotics is approximately two weeks, but it may take up to three weeks.
Once your custom orthotics are ready, our office will contact you to schedule a fitting session with one of our healthcare professionals. During this fitting session, we will ensure that your orthotics fit correctly and comfortably, and we will make any necessary adjustments to optimize their effectiveness. At Fisiomed, we strive to provide a comprehensive approach to care, and this includes ongoing support to ensure your continued comfort and success with your custom orthotics.
